If the cruise is advertised as "fly/cruise" then all flights will be included and as previous answer if long haul flight overnight hotel acommodation sometimes included - I am doing Panama in January and hotel included both going out and coming back.
If you book as package, you can book cruise and flight separately and orgainse your self however, delays and cancellations may cause you problems. Make sure that your transfers are also included, not all operators provide them in the package. We also had a hire car included in last fly/cruise/stay package but be sure you have all the relevant paperwork and vouchers before travelling or you will incurr the cost.

Yes, apart from silverseas cruises ,the seats are in ecommany if you are flying any distant I would pay for an upgraded seat it makes all the difference.
